Documents Needed for minor piercings

-Valid State or Government Issued Photo ID
-Birth Certificate

-Parent on said Birth Certificate with valid State or Government Issued ID.
   (Legal guardianship papers accepted)

-Must be 14+ for body piercing
-Must be 5+ for lobe piercings (child ID not required)
-No piercings below the neck for minors with exception of Navel
